After the untimely death of the much-liked Emperor Trajan, the despotic Emperor Arruntius has bled the once powerful Rome to its knees. Badly fought wars, famine and plague are rife in all corners of the Empire, whilst the Emperor and his elite enjoy every excess and vice that the emptying coffers can afford. As herald to this age of degradation, Arruntius has destroyed large portions of the city of Rome to create the battleground for the bloodiest gladiator event ever staged.

Året er 106 e.Kr. og Romerriket står for fall. Etter at keiser Trajan dør, lever den despotiske etterfølgeren hans, keiser Arruntius, et liv i luksus (det samme gjelder mesteparten av Roma), og river ned store deler av Roma for å bygge en gladiatorarena verden aldri tidligere har sett maken til. Til denne arenaen kommer Invictus Thrax, som er født slave og har blitt oppdratt til et liv som gladiator. Men ikke en gang Thrax, med sine sylskarpe sverd og susende kombinasjoner, kan stå i mot keiserens menn for evig og alltid, og når han faller, ender han opp i det hinsidige paradiset Elysium.

Her får Thrax i oppdrag å sette en stopper for Arruntius' herredømme ved hjelp av Romulus og Remus, som selv handler på vegne av gudene. Thrax vender dermed tilbake til jorda, nå utstyrt med nye evner og kombinasjoner, og alt ligger til rette for et blodig eventyr av et beat 'em up-spill.

I Gladiator: Sword of Vengeance er vekten lagt på blodig action, fart og spenning, og Thrax hugger og kutter seg vei gjennom en hær av motstandere. Med kombinasjoner som er relativt enkle å mestre og "henrettelsesmanøvrer" som er tilgjengelige med et enkelt knappetrykk, appellerer dette til dem av oss som ikke helt fikser kompliserte beat 'em up-spill, men som likevel hungrer etter blod og gørr i den stilen som Mortal Kombat ga oss.
